How to Reach Us — In Order of Preference

Please use these channels in the order shown below. Each one is appropriate for the situation listed next to it.

For emergencies, call 911 or 988 (mental health). Do not use any of the above channels for emergencies.

Schedule a Visit

The portal is the easiest place to schedule. You can also call, text, or email.

Office hours: Monday–Friday, 9 AM–5 PM Pacific.

New members — your first visit is a 60- to 90-minute evaluation; let us know preferred days/times.

Outside Office Hours

For non-urgent questions, send a portal message, text, or email. We’ll respond during office hours.

For urgent but non-emergent concerns after hours, use an urgent care center or telehealth urgent-care service.

For medical emergencies (chest pain, shortness of breath, stroke symptoms, severe bleeding), call 911 or go to the nearest emergency department.

For mental health emergencies, including suicidal or homicidal ideation, call or text 988 (Suicide and Crisis Lifeline), call 911, or go to the nearest emergency department.

Labs — Important

Lab orders are sent through your patient portal. Bring the printed requisition (or have it on your phone) to your Quest visit.

We have a wholesale account with Quest under Account #73950133, ATLAS group pricing. The PSC should bill the practice (Client Bill) at the group rate, not bill you at retail.

If anything seems off at the PSC — they can’t find your order, they quote you a price that feels wrong, or they ask for any payment beyond a small specimen-handling fee — STOP and call my cell at (651) 285-2411 immediately before you sign anything or pay. Don’t complete the draw under retail/walk-in billing. I can usually resolve it in 5 minutes with our Quest rep.

Sending Records from Prior Providers

To bring your prior medical records into your chart, ask your previous providers to fax records to (279) 239-5776 — the practice fax. Faxed records auto-route into your chart in the portal.

You can also upload records directly through the patient portal as you receive them.

A useful request to make: “Please fax my recent labs, imaging reports, problem list, medication list, and consultation notes from the last 2–3 years to (279) 239-5776, attention Dr. Sahni at Private MD.”

Prescriptions

Prescriptions are sent directly to the pharmacy of your choice — Walmart, CVS, Walgreens, Costco, your local pharmacy, anywhere. Just let us know which one.

For wholesale generic pricing, ask about Cost Plus Drugs and Amazon Pharmacy — both can ship medications to your home at a fraction of retail.

What Your Membership Covers

Your membership covers the services listed in your signed agreement, subject to clinical appropriateness, scheduling, and practice capability. Refer to your agreement for the full list of included services and what falls outside the membership.

Care outside the membership scope — hospital, ER, outside specialists, advanced imaging, surgery, and most prescription medications — is the Member’s responsibility. You’re encouraged to maintain separate health insurance for catastrophic medical care.
