Your Physician
Nishant Sahni, MD, MS
Founder & Physician · Internal Medicine · Clinical Informatics
“I spent two decades in academic and hospital medicine — teaching residents, managing critically ill patients, running research programs. But the system that trained me was also the system that gave me 15 minutes per patient and 2,000 names on a panel. I started this practice to do what I was trained to do: know my patients, think carefully, and be there when it matters.”
Dr. Sahni is a board-certified internist who served as Associate Professor of Medicine at Mayo Clinic (2020–2022) and as Assistant Professor at the University of Minnesota Medical School (2012–2020). He is board-certified in Internal Medicine and Clinical Informatics, holds a Master's degree in Molecular Immunology, and speaks English, Hindi, Urdu, and Punjabi. His clinical research has appeared in the Journal of General Internal Medicine, Mayo Clinic Proceedings, Critical Care Medicine, and other peer-reviewed journals (full list on PubMed). His work on AI-assisted clinical decision-making was covered by the Minnesota Star Tribune, and he holds a US patent for a clinical decision-support algorithm licensed through the University of Minnesota's Technology Commercialization office. At Private MD, he focuses on bringing comprehensive, beyond-routine screening into primary care.
Outside the office, Dr. Sahni enjoys time in nature, playing guitar, and reading.
What This Looks Like in Practice
- A direct relationship with your physician. Because the panel is small, your physician can answer messages directly — no phone tree, no portal callback queue, no front-desk gatekeeping for clinical questions.
- Timely office visits. A 300-patient panel typically means same-day or next-day availability — not the 2–6 week wait common in traditional primary care.
- Same-day responses to clinical questions during business hours. Most days the response is within a few hours.
- Unhurried visits. Because we don’t double-book, visits run as long as the conversation needs — not on a 12-minute clock.
- Continuity. You see the same physician every time. Your story doesn’t restart at every visit.
- Time to think. Diagnostic and treatment decisions made with room to reason carefully — not between charting tasks.

Questions We Often Hear
Is this concierge medicine?
Yes — same small-panel feel, with two clear tracks. Direct Primary Care covers everyday primary care directly, no insurance involved. Concierge Internal Medicine is for members who want comprehensive screening for heart, cancer, brain, and metabolic disease, with state-of-the-art testing and a written annual care plan. Available standalone or as an add-on to DPC.
Do you accept insurance?
Our membership model isn’t insurance-based, but we work with Medicare patients in the Concierge program. The specifics are easiest to walk through on a short call.
Can I use my HSA or FSA?
Yes. As of 2026, DPC memberships up to $150/month are fully HSA-eligible. Our membership ($149/mo) qualifies in full. The $299 enrollment (initial evaluation + first two months) is also HSA/FSA eligible.
Do I still need health insurance?
Yes. Maintain a health plan for hospital stays, emergencies, specialists, and prescriptions. Many members pair this membership with a high-deductible or catastrophic plan.
Can I cancel my membership?
Yes. There is no long-term contract. You may cancel at any time with 30 days' written notice. There are no cancellation fees.
