Founded by two physicians who both taught and practiced at Mayo Clinic and the University of Minnesota, this practice was created to restore what medicine should be: physicians who are available, know you personally, and have the time to care for you properly.
Nishant Sahni, MD, MS
Founder & Physician · Board-Certified Internal Medicine
After nearly two decades in academic and hospital medicine — including faculty positions at Mayo Clinic and the University of Minnesota, where he taught residents, ran clinical research, and cared for critically ill patients — Dr. Sahni faced a question every academic physician eventually confronts: keep seeing 2,000 patients a year in 15-minute slots, or build something where he could actually practice the medicine he was trained to deliver.
He chose to start over. Private MD is the result — a practice small enough that he knows every patient by name, their history, their medications, and what matters to them. He holds a Master's in Molecular Immunology from UT Southwestern, is board-certified in both Internal Medicine and Clinical Informatics, and is an extensively published researcher. He speaks English, Hindi, Urdu, and Punjabi.
Rashi Arora, MD
Physician · Board-Certified Internal Medicine
Dr. Arora spent over two decades as a hospitalist at Mayo Clinic and the University of Minnesota — leading inpatient teams, managing critically ill patients, and training residents. That experience gives her a depth of clinical judgment that most outpatient physicians simply don't have.
Her clinical focus includes women's health — perimenopause, menopause, hormone management, and cardiovascular prevention in women — alongside the full scope of internal medicine. She speaks English, Hindi, Urdu, and Punjabi.
Dr. Arora is not accepting patients at present.

What is Direct Primary Care?
A membership-based practice where you pay your physician directly through a monthly fee rather than billing through insurance. This removes administrative overhead and allows your physician to maintain a smaller patient panel and spend more time with each patient.
Do you accept insurance?
No. This is a membership model, not an insurance-based practice. You pay a flat monthly fee that covers all primary care visits and direct physician access. You will receive itemized receipts that can be submitted to your HSA or FSA for reimbursement.
Can I use my HSA or FSA?
Yes. As of 2026, DPC memberships up to $150/month are fully HSA-eligible. Our membership ($149/mo) qualifies in full. The $299 enrollment (initial evaluation + first two months) is also HSA/FSA eligible. You’ll receive itemized receipts for everything.
Do I still need health insurance?
Yes. This membership covers your primary care — visits, labs, direct access to your doctor. You should maintain a health plan for hospital stays, emergencies, specialist visits, and prescriptions. Many members pair this membership with a high-deductible or catastrophic plan and find their total annual cost is lower than a full-coverage plan alone. The membership handles the care you actually use; the health plan covers what it should: major events. For most healthy adults, the math works out.
My health insurance premiums went up — does a membership make sense?
It might. Health insurance premiums are climbing again, and most of what you pay for — the office visits, the labs, the routine care — doesn’t require insurance at all. Our members pay $149/month for all of their primary care: same-day visits, wholesale labs, and direct access to a board-certified internist. Paired with a catastrophic or high-deductible plan for hospitalizations and emergencies, the total is often significantly less than a full-coverage plan — and the care is incomparably better. We’re happy to walk you through the numbers during a free introductory conversation.
Do you have a physical office?
Yes. We offer in-person visits at our El Dorado Hills location as well as virtual and phone visits when you don't need to come in. Your physician will recommend an in-person visit when a physical exam is needed.
Do you prescribe GLP-1 medications like Ozempic, Wegovy, Mounjaro, or Zepbound?
Yes. We prescribe semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound). For members with type 2 diabetes, insulin resistance, or other metabolic conditions, GLP-1 prescribing and monitoring are included in your membership. For weight management, we offer a dedicated GLP-1 weight management program ($99/mo add-on for members, $249/mo standalone) with medical evaluation, monthly monitoring, dose titration, and metabolic lab work. Medications are prescribed and filled through licensed pharmacies at cash-pay pricing.
What about my records from another doctor?
Many of our members transition from Kaiser or other health systems. We can request records from your previous doctors electronically or by fax. For systems like Kaiser that use separate patient portals, we ask that you download your records and bring them to your first visit. Your physician will review everything and build your care plan from there.
How much do labs cost?
We order labs at wholesale physician rates through Quest and LabCorp — both located near our office. A typical annual wellness panel (metabolic, cholesterol, thyroid, blood count) costs $17–25 through us, compared to $200–400 at retail cash prices. You pay only our wholesale cost with no markup. Your membership includes the ordering and interpretation — you just pay for the lab draw itself.
What about imaging costs (MRI, CT, X-ray)?
If you have insurance, imaging is billed through your plan as usual. For uninsured patients or those on high-deductible plans who want to stay under their deductible, we route to local cash-pay imaging centers where prices are typically 50–80% less than hospital outpatient rates. Your physician orders exactly the imaging you need, coordinates with the center, and reviews the results with you directly.
Can you help with medication costs?
Yes. We find the lowest price for every prescription. For most generics, we direct you to pharmacies like Cost Plus Drugs or Amazon Pharmacy, where common medications like blood pressure, cholesterol, and diabetes drugs cost $4–8/month instead of $20–40 at retail. For same-day needs, we’ll help you find the best discount at a local pharmacy. For GLP-1 and specialty medications, we work with manufacturer programs (NovoCare, LillyDirect) to find the lowest available price.
If a medication requires prior authorization from your insurance, we write and submit the clinical justification directly — no phone trees, no runaround, no weeks of waiting.
What is not included?
This membership does not cover hospital care, emergency room visits, specialist referrals, advanced imaging, or prescription medications. Procedures like colonoscopy and mammography are coordinated by your physician but performed at outside facilities. Lab work is available at wholesale pricing through Quest and LabCorp.
The GLP-1 weight management program is available as an add-on ($99/mo for members, or $249/mo standalone). Hormone evaluation and management (TRT, HRT) are handled as part of your primary care when clinically indicated — no extra fee for members.
Can I be seen without a membership?
Yes. A one-time consultation is $300 and includes a full visit with one of our physicians. No membership required. If you'd like to schedule one, let us know and select “One-Time Visit.”
Do you offer home visits?
Yes, for members in El Dorado Hills. Home visits are available for patients with mobility limitations or those who prefer care at home. Pricing is discussed at enrollment. If this interests you, let us know and select “Home Visit.”
Can I cancel my membership?
Yes. There is no long-term contract. You may cancel at any time with 30 days' written notice. There are no cancellation fees.
Do you see children?
This practice serves adults aged 18 and older. Internal medicine focuses on adult health, including complex and chronic conditions. For pediatric care, we can recommend a trusted provider in the area.
