Nishant Sahni, MD, MS

Dr. Sahni is a board-certified internist with over two decades of experience in academic and hospital medicine. He taught and practiced at Mayo Clinic and the University of Minnesota, where he served as Associate Professor of Medicine — training physicians, leading clinical research, and managing critically ill patients at the bedside.
He created this practice to offer a different kind of care — one where physicians have the time to listen, think carefully, and build long-term relationships with their patients. He speaks English, Hindi, Urdu, and Punjabi.
Rashi Arora, MD

Dr. Arora is a board-certified internist and hospitalist with over two decades of experience in academic and hospital medicine. She taught and practiced at Mayo Clinic and the University of Minnesota, where she served as Associate Professor of Medicine — training physicians, leading inpatient teams, and managing complex cases at the bedside.
She co-founded this practice to bring the same rigor and attention to outpatient care that she brought to the hospital — where physicians have the time to understand the full picture, not just the presenting complaint.

Questions We Often Hear
What is Direct Primary Care?
A membership-based practice where you pay your physician directly through a monthly fee rather than billing through insurance. This removes administrative overhead and allows your physician to maintain a smaller patient panel and spend more time with each patient.
Do you accept insurance?
No. This is a membership model, not an insurance-based practice. You pay a flat monthly fee that covers all primary care visits and direct physician access. You will receive itemized receipts that can be submitted to your HSA or FSA for reimbursement.
Can I use my HSA or FSA?
Yes. As of 2026, DPC memberships up to $150/month are fully HSA-eligible — you can pay your membership from pre-tax HSA funds while continuing to make HSA contributions. The initial evaluation is billed as a medical service and is also HSA/FSA eligible. You'll receive itemized receipts for everything.
Do I still need health insurance?
Yes. This membership covers your primary care. You should maintain a health plan for hospital stays, emergencies, specialist visits, and prescriptions. Many members pair this membership with a high-deductible health plan and find their overall costs decrease.
Do you have a physical office?
Yes. We offer in-person visits at our Folsom location as well as virtual and phone visits when you don't need to come in. Dr. Sahni will recommend an in-person visit when a physical exam is needed.
Do you prescribe GLP-1 medications like Ozempic or Zepbound?
Yes. For members with a medical indication — type 2 diabetes, insulin resistance, or other metabolic conditions — GLP-1 prescribing and monitoring are included as part of your standard membership. For weight management without a primary metabolic diagnosis, we offer a separate GLP-1 program that includes medical evaluation, monthly monitoring, dose titration, and metabolic lab work. Medications are ordered at cash-pay pricing directly from the manufacturer and shipped to your home.
What about my records from another doctor?
Many of our members transition from Kaiser or other health systems. For hospitals on the Carequality network — including UC Davis, Sutter, Dignity, and Stanford — records flow into our system automatically. For systems outside that network, such as Kaiser, we ask that you download your records from your patient portal and bring them to your first visit. Dr. Sahni will review everything and build your care plan from there.
What is not included?
This membership does not cover hospital care, emergency room visits, specialist referrals, advanced imaging, or prescription medications. Procedures like colonoscopy and mammography are coordinated by Dr. Sahni but performed at outside facilities. Lab work is available at wholesale pricing through Quest and LabCorp.
Can I be seen without a membership?
Yes. We offer one-time visits for patients who are not ready to commit to a membership. These are priced as individual consultations and are a good way to experience the practice. If you'd like to schedule one, let us know and select “One-Time Visit.”
Do you offer home visits?
We are exploring home visits for members who need them — particularly for patients with mobility limitations or those who prefer the convenience of care at home. If this interests you, register your interest and select “Home Visit.”
Can I cancel my membership?
Yes. There is no long-term contract. You may cancel at any time with 30 days' written notice. There are no cancellation fees.
Do you see children?
This practice serves adults aged 18 and older. Internal medicine focuses on adult health, including complex and chronic conditions. For pediatric care, Dr. Sahni can recommend a trusted provider in the area.
Do you accept Medicare patients?
We are not currently enrolling Medicare patients, but we are exploring this for the future. If you are on Medicare and interested in this style of care, please register your interest and select “Medicare Patient” — it helps us plan.
