That means you actually get your doctor when you need one.
With Private MD, appointments are available unhurried, visits are never rushed, and you have continuity to Dr. Sahni for questions between appointments. For routine follow-ups and minor concerns, virtual visits are available so you can get care from the comfort of your Granite Bay home.

Our Two Programs
Direct Primary Care — $149/mo
For adults under 65. Comprehensive internal medicine in an unhurried, small-panel practice. Up to 30 visits per year included; $25 per visit thereafter. Includes annual cardiometabolic labs (ApoB, Lp(a), fasting insulin, hs-CRP), wholesale lab and medication pricing, and the full scope of internal medicine — chronic disease management, preventive care, acute illness, diagnostic evaluation, mental health, and men's and women's health.
Concierge Longevity Program — $299/mo · couples discount available
For adults 65+, or available as an add-on at any age. Everything in Direct Primary Care, plus the longevity medicine layer: annual Longevity Assessment, interpretation of multi-cancer screening (Galleri), pharmacogenomic wellness profiling, wearable data synthesis, non-diabetic CGM pattern analysis, longevity labs, DEXA body composition, whole-body MRI interpretation, epigenetic age clocks, coronary plaque imaging (Cleerly AI), and a personalized Longevity Blueprint. For Medicare-enrolled members, Medicare is billed normally for all covered clinical services; the membership fee is allocated exclusively to non-covered longevity services.
GLP-1 Weight Management
Private MD offers physician-supervised weight management with GLP-1 medications including semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound). The program includes medical evaluation, monthly monitoring, metabolic lab work, and physician-led titration. Follow-up visits can be done virtually from Granite Bay.
