Advanced Cardiac Prevention — see your risk before symptoms.

A physician-led prevention program combining AI-quantified coronary plaque imaging, advanced lipid testing (ApoB, Lp(a)), and lifestyle medicine. Serving Folsom, El Dorado Hills, Granite Bay, Roseville, and the greater Sacramento area.

Cleerly AI cross-sectional analysis of a coronary artery showing non-calcified (soft) plaque highlighted in yellow around the patent lumen — the type of plaque that causes most heart attacks but is invisible to a standard calcium score
Non-calcified (soft) plaque, highlighted in yellow, surrounding the artery lumen. This is the type of plaque most likely to rupture and cause a heart attack — and the type a standard coronary calcium score cannot detect. Learn more about Cleerly imaging →

From Dr. Sahni

Heart disease is still the number one killer in the United States — and the standard primary care approach is mostly reactive. We measure your LDL, run a 10-year risk calculator, and either prescribe a statin or watch and wait. That works for some people. For many others, by the time symptoms appear, the disease is already advanced.

I built this program because I wanted patients to have something better. Advanced lipid testing tells us what your LDL number alone can't. AI-quantified plaque imaging shows us whether disease is already there. And a physician who actually reads the imaging in the context of your whole risk profile, not one who just orders the scan and moves on.

What's in the program

Who this is for

How it works

  1. Enroll at Signature or Executive — the Advanced Cardiac Prevention Program is included. See pricing.
  2. Initial assessment — full history, advanced lipid panel ordered, baseline risk modeling, family history detail.
  3. Imaging if indicated — Cleerly CT scan scheduled at Valley MRI Lodi ($1,600) or RadNet Sacramento ($2,200), both Cleerly-integrated.
  4. Interpretation session — we sit down together (in person or virtually), walk through your imaging and labs, and build your individualized prevention plan.
  5. Ongoing care — repeat labs at intervals appropriate to your risk, medication management if applicable, lifestyle support, recalibration as your data evolves.

Why this is different from your annual physical

A standard physical is built around what insurance pays for: basic cholesterol, blood pressure, glucose, a 10-year risk calculator. It's necessary but rarely sufficient — especially for people whose actual cardiovascular risk is hiding behind a "normal" LDL number.

What we measure instead: ApoB (better than LDL at identifying the particles that cause disease), Lp(a) (genetically determined, missed by every standard physical, identifies the 1-in-5 adults with elevated genetic risk), and when it’s worth doing, a look at the arteries themselves.

The standard physical tells you what your numbers are today. This program tells you what's actually happening in your arteries — and what to do about it.
